Physical Therapist

Physical Therapists provide Physical Therapy to clients based on an evaluation’s goals. Physical Therapists complete daily notes to produce progress reports, coordinate with supervisor to update, modify, and remove client goals.

REQUIREMENTS & QUALIFICATIONS:

  • Graduate of an accredited Occupational Therapy Assistant (OTA/COTA) program.

  • State licensed to work as a COTA in Arizona.

  • Driver’s License or State Issued ID Card

  • Social Security Card or Birth Certificate

  • 3 References

  • First Aid Certificate

  • CPR (Cardio-Pulmonary Resuscitation) Certificate

  • Article 9. Managing Inappropriate Behaviors

  • Level One Fingerprint Clearance Card

If you’re missing or expired for any of the items listed above we will sign you up and even help to pay for the training you need for:

(we’ll help you sign up but you will be responsible to pay the $75 fee)

**You certainly may interview and move forward with the hiring process without the above required items but you will not be able to start working until you have obtained them.**
